Is this a good line for my client intake form?
I’m creating an intake form for therapeutic massage clients. Is this a good, clear instruction? :
“Please fill out this form in order for your massage therapist to be well informed about your medical background and be capable of giving you the safest care during your massage.”
Please let me know if there are any mistakes or better options.

Too long..how about:
“Our goal is to provide the safest care possible during your massage. In order to ensure a high level of care, please fill provide the following information.”
To make it more personal, change “Our” to your company’s name, for example, “The goal of Tina’s Massage is…”
